How to Download Tax Form 1095-A from your Covered California Account?

Question: I would like to know what website I can go to, to print out my Tax Form 1095A? Can I print it online? Please advise.

Answer: The web address for the Covered California Account Login is: https://apply.coveredca.com/apspahbx/login.portal

If you don't have a login and password call 800-300-1506.

After logging in, you'll be on the Consumer Home Screen. Look for the list of links in the bottom-center of the page.

Click on Eligibility Results
Click on Summary
Click on Documents and Correspondence
Click on _IRSForm1095A_2017

I have spent 8 hours on and off on the phone with Covered Ca agents, none were able to help me to get the 1095A form. Since I’m not an active member they have closed my account and then tried to get a new account the Agent found 3 accounts under my name and was not able to help me at all. Instead, he told me to contact the agent that enrolled me in the plan after I did that, my Agent stated he could not access my acct either. Then called for the 5th time to cover calif and stayed on hold for 45 min till an agent did the same steps of creating a new account, and again was not successful and told me to go to the provider clinics, game me 2 addresses and phones. When I called them they stated they couldn’t help me that they never printed any forms for cover calif and other places no longer exist with wrong telephone numbers. I’m so frustrated because of my taxes are on hold until I get that form. Is any experiencing any of this problems? and how they went through getting this stupid 1095A form?

Does not work for me. Eligibility Results leads me nowhere, no button for summary. I saw a documents & Correspondence link somewhere else on the website however there was nothing within the past 7 months in there. Currently receiving silver premium assistance so I know I’m in the system. Their youtube video (with comments disabled, of course) provides no help to those that never received the message in the first place. Hopefully going into a CoveredCA center/brick and mortar business will help, otherwise I’ll have to take a day off work to wait on hold.

We had Insurance with Covered Ca up to sep when we switched to my wifes health insurance. I cannot find my 1095-a and I can no longer log in to download as our account has been closed. How can i get a copy of our 1095-a?

I am a single mother with two children. My daughter is 13 and my son is 4 I have to turn in my review by March 31st based on the income limits I do not qualify currently, I now make 1,370 bi weekly. However I see the new income limits start on April 1st 2018, based on this information do I qualify? Should I reach out to my worker and see if I can extend my turn in date for my recertification? Or should I just apply all over again April 1st
