How to Submit Paper Application?

Question: Since all licensed agents are temporarily certified for October, we are taking paper applications. What is the fax number to submit these applications?

Answer: Licensed agents are NOT temporarily certified for October. Sorry, but I take the lions’s share of the blame for publicizing that fallaciousness. So don’t submit any business to Covered California until they notify you that you are certified and are given a unique identifying number. Fax to 888-329-3700 Mail to: Covered California, P.O. Box 989725, West Sacramento, CA 95798-9725.


I have bene trying to enroll for the past hour. I have to go to work I can’t keep trying to do this. I tryed printing the application so I copuld do it paper. I cant find the paper application

Where do I submit documents that are required by mail. I need an address to mail them to.

The information above saying the fax # given is not a fax # is totally incorrect.

I have successfully sent faxes there as has my insurance agent.

Furthermore I just called it and received a fax tone when it picked up.

I just tried to fax also and the number was not working for faxing, so I called on my landline and a man answered who said it was not a fax number. Very strange. I DIALED 1-888-329-3700

You can fax Individual applications to Covered Ca. The fax number is 1-888-329-3700 as stated on page 29 of the Covered Ca application I faxed an application yesterday.

One correction. I do not believe Covered CA is assigning unique numbers to certified insurance agents. I am now certified and have not received such a number. In addition, on the paper application it requires my insurance license number for identification, not a new number specific to Covered California.
